Social Worker

Full Time Days

** Salary between $56,584 - $63,656/year! **

UMC is currently seeking a Social Worker for our Bristol Glen Senior Care Community located in Newton, NJ. Bristol Glen, a Continuing Care Retirement Community (CCRC), is thoughtfully designed to provide person directed care in a home-like environment to ensure exceptional care and quality of life for our residents.

The Social Worker oversees and assists our Healthcare residents by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.

Requirements for a Social Worker:

  • Bachelor's degree in Social Work (BSW) or Human Service field.
  • Proficiency in a clinical software system.
  • Understanding of the MDS, care plan and admission and discharge processes.
  • One year healthcare/long term care experience.
  • Certified social worker license with the State of New Jersey (CSW).

Responsibilities for a Social Worker:

  • Develop, maintain and utilize a listing of current community resources.
  • Advocate for the resident and their family.
  • Understands and meets all requirements for social service documentation, i.e. interdisciplinary care plan; d/c notes: advanced directives; resident rights, etc.
  • Prevents and addresses resident abuse.
  • Educate associates regarding residents' rights including how to recognize and prevent abuse, neglect and mistreatment, resident dignity, independence and quality of life - as outlined in the Resident Rights.
  • Provide the residents, families, significant others and associates with communications regarding care conferences such as the date and the time of the meeting.
  • Perform other duties as assigned or as responsibilities dictated.
