Location: Zuni, NM & Gallup, NM (3 days in Zuni / 2 days in Gallup)

Position Summary:

Our client is seeking a dedicated Social Worker to join a multidisciplinary care team serving patients living with chronic kidney disease. In this role, you will support patients’ psychosocial well-being and adjustment to dialysis treatment by addressing emotional, social, and practical challenges. You’ll also help coordinate support resources and advocate for patients to ensure they receive holistic, high-quality care.

What You’ll Be Doing:

Advocacy & Support:

• Conduct psychosocial assessments for new and current patients, including completion of the KDQOL (Kidney Disease Quality of Life survey) in accordance with organizational policy and CMS/state regulations.
• Provide individual and family counseling to support psychological adjustment to dialysis.
• Educate patients and families on available resources and coping strategies.
• Identify community resources, social agencies, and financial assistance programs to support patient needs.
• Coordinate and advocate for transient care arrangements and work with local, state, and federal agencies on behalf of the patient.

Team Collaboration:

• Act as a key member of the interdisciplinary care team alongside physicians, nurses, dietitians, and patient care staff.
• Promote a collaborative team environment and help educate staff on psychosocial aspects of patient care.
• Attend all required staff meetings and continuing education sessions.
• Maintain productive relationships with local hospitals, community agencies, vendors, and support services.

Safety & Quality:

• Support initiatives to reduce missed treatments and hospitalizations through early identification of psychosocial barriers.
• Participate in Quality Assessment and Performance Improvement (QAPI) programs and other clinical/operational efforts to enhance patient outcomes.
• Ensure compliance with all applicable federal, state, and local laws and regulations .

Work Schedule:

• Standard Hours: Monday to Friday, 8:00 AM to 5:00 PM
• Location Split: 3 days in Zuni and 2 days in Gallup
• Flexibility may be required based on clinic needs

Qualifications:

Required:

• Master’s Degree in Social Work (MSW) from an accredited institution
• Current state licensure as a Social Worker (LMSW, LCSW, or equivalent based on state)
• Excellent communication, assessment, and documentation skills
• Ability to travel between clinics (Zuni and Gallup)

Preferred:

• Experience in dialysis, nephrology, or chronic disease management
• Knowledge of CMS regulations and QAPI processes
• Bilingual skills (Spanish or Native American languages) a plus, depending on patient population

Note: The hourly pay rate and potential sign-on bonus will be discussed directly with the Hiring Manager during the interview process.
