Job Description

Blize Healthcare is looking for a Medical Social Worker to join our team. You will help patients and their families navigate the emotional, social, and financial challenges associated with a medical diagnosis or illness. Your role is to ensure patients' needs are met, promote health, and prevent disease.

Responsibilities:

• Evaluating the psychosocial functioning of patients and families.
• Providing individual or group counseling to address emotional, behavioral, and social issues.
• Coordinating care, connecting patients with resources, and advocating for their needs.
• Helping patients and families prepare for transitions from hospitals to home or other settings.
• Assisting patients in accessing benefits, support services, and other resources.

Required Skills and Qualifications:

• Master's degree in Social Work from an accredited program.
• Home Health or Hospice experience strongly preferred.
• Strong communication and interpersonal skills.
• Empathy, patience, and a deep sense of service.
• Physical strength and ability to lift up to 50 pounds, as needed.

License/Certification:

• Valid BLS/CPR certification.
• Driver's License.
